Services

An initial discussion with you, your family, and/or hospital care team to determine care needs and support a smooth transition home.  Available to meet you at home at time of discharge and includes up to 3 RN home visits per day to provide required nursing care/assessments, medications, equipment, wellness checks, updating family and alerting your Dr./ care team as needed.  Care is customized to provide you with what you need. 1 day and 2-day packages available. $300/day

Additional hours or overnight care may be available for an additional fee. 

A comprehensive in-home assessment and action plan to identify and hear your areas of concern such as depression, memory issues and home safety. Together we will create an action plan that will help support you to live at home safely and with the supports you need.  $150. 

Ongoing support to assist with carrying out action plan available for an additional fee.
